If your business needs a Single Family Plan (you have no employees other than your spouse or children), then please complete that enrolment form and mail it to us.
If your business has employees, you need a Managed Plan. Please contact us to set up a Managed Plan for your employees.
An enrolment fee of $100 is charged to register a new plan.

Once received, please allow up to two weeks to process the enrolment. HealthSmart will then send you an enrolment information package, including individual subscriber identification cards for each covered employee.
